A place to recover after discharge

I'm Aoi Hashima , an occupational therapist working in Saga Sumai and Fukuoka.

I think the general image of being admitted to an emergency hospital after a stroke, then admitted to a recovery ward, and then discharged home or admitted to a facility is becoming more common.

If the brain damage from the stroke is severe, or if the symptoms of the stroke are so severe that living at home may become difficult, you may be admitted to a medical facility.

After the completion of inpatient rehabilitation after stroke, some people need to continue rehabilitation after discharge, and some do not need to continue rehabilitation.

If recovery is required after discharge, we discuss it in a pre-discharge meeting, where the medical advisor will introduce the post-discharge recovery destination while understanding the intentions of the patient and their family.

This time, I will introduce you to the places where you can recover after being discharged from the hospital.

Medicare Rehab

▶︎Outpatient Rehabilitation
Outpatient rehabilitation is available if less than 180 days have passed since the stroke onset .

If more than 180 days have passed since the onset of the stroke, the number of outpatient recoveries per month will be limited.

Outpatient rehabilitation after 180 days of stroke onset was limited to 13 units per month (20 minutes per unit).

If you do not have long-term care insurance, outpatient rehabilitation is available (partially combined).

For outpatient rehabilitation, some hospitals do not provide 13 units of rehabilitation, please contact each hospital.

▶︎Home Visit Rehabilitation
Home Visit Rehabilitation is available if outpatient rehabilitation is difficult and the doctor recognizes the need for rehabilitation.

If you have long-term care insurance, you are not eligible for home visit rehabilitation services under Medicare.

Rehabilitation with long-term care insurance

▶︎Day Care
If you have long-term care insurance, there is a rehabilitation service that allows you to take a bath from your home to the facility or while you are using the facility.

Day care facilities are legally staffed with physiotherapists, occupational therapists and speech-language pathologists.

Individual rehabilitation takes place within the rehabilitation implementation time established by the facility, while using several hours of day care.

Depending on the facility, if there are enough training machines, you can not only undergo personal rehabilitation, but also machine training.

▶︎Day service
Day service is the same as day care, if you have long-term care insurance, you can use it.

A facility car will pick you up from your home and you will receive rehabilitation while using the facility.

Unlike day care, day services are not legally obligated to have physical therapists, occupational therapists and speech-language pathologists.

Nurses qualified as functional training instructors assemble rehabilitation menus, but the number of physical therapists working in day care services has increased in recent years, making it difficult to understand the clear difference between day care and day care. I am coming.

▶︎Home Visiting Rehabilitation
If you have difficulty going to a day care center or other type of rehabilitation center, and your doctor thinks you need rehabilitation, you can choose Home Visiting Rehabilitation, which allows you to receive rehabilitation at home.

My impression is that for those who are not good at spending time in daycare or day service groups, or when they are focusing on improving their movement at home, many use home visiting rehabilitation.

Rehabilitation without Insurance

▶︎ Self-Expenses Rehabilitation
Rehabilitation without Medicare or Long Term Care Insurance.

Since I don't use insurance, there is no limit to the number of strokes per month, even 180 days after the onset of the stroke.

Medicare and Nursing Insurance, 10% to 30% of the statutory amount is paid to hospitals and facilities, but in the case of out-of-pocket rehabilitation, you must pay the full amount yourself.

Compared to rehab without insurance, the amount will be higher, but my impression is that there are many facilities with highly personalized rehab and a wide range of rehab equipment.

At the end

This time, we summarize where you can recover after discharge.

I would be grateful if you could help me.
