About falling after a stroke

I'm Aoi Hashima , an occupational therapist working in Saga Sumai and Fukuoka.

After a stroke, some people may fall while walking.

Also, I don't think some people fall while walking.

Endo et al. (1997) reported that people who fell multiple times all fell with the same movements and believed that knowing where and what movements might lead to fall prevention would lead to fall prevention.

This time, I wanted to write a story about a fall after a stroke.

where did you fall

Falls in hospitalized patients after stroke occur in different places.

According to Endo et al., the place where the person fell is

✔️ Recovery room

✔️ Ward

✔️ Corridor

✔️ toilet

The report says yes.

10% of falls occurred in recovery rooms, and the rest occurred in wards, corridors, etc.

What sport did you fall in?

According to Endo et al., the behavior when falling is

✔️ While walking

✔️ When you try to sit down

✔️ When you try to stand up

✔️ When trying to move between the wheelchair and the bed or toilet

✔️ While sitting

The report says yes.

It can be seen that the behavior when falling is when there is a large change in motion, such as a change in posture or a change in the center of gravity, rather than when sitting.

Physical difference

We tend to think that people who can walk independently do not fall, but even people who can walk independently are at risk of falling.

Takamine (2005) investigated the ADL status of people who fell and wrote that falls were reported regardless of whether they were wheelchair users, walking assistance levels, or independent walking levels.

Interestingly, in terms of the number of falls, the results showed that the number of falls was higher at the level of independent walking than at the level of assisted walking.

If walking at assist level and you fall, you can imagine that you will walk alone and fall.

Also, I feel the need to understand that even when walking independently, it is possible to fall due to various triggers (carelessness, accident, panic, etc.).

Many reports of fall factors can be corroborated.

The fall factor dendrogram presented by Nihei et al includes other diseases than stroke, but can list various factors associated with falls.

The causes of falls shown in the treemap above include:

① Sickness/Disability

② Sensory factors

③ Exercise factor

④ Higher brain function factor

⑤ Behavioral factors

⑥ Psychological factors

etc. are covered, the main factor for falls is

✔️ Ability to avoid obstacles

✔️ Balance ability

✔️ Action frequency

✔️ fainted

I am improving it.


From these results, it can be seen that falls are not caused by a single factor, but by multiple factors.

At the end

Fall prevention is important for living safely, but it is also important for preventing secondary injuries (broken bones, etc.) due to falls.

It is important to use transportation and mobility that is appropriate for your physical condition.
